Charlotte Hungerford Hospital is a 122-bed, general acute care community hospital located in Torrington, Connecticut, that serves as a regional health care resource for the 100,000 residents of Litchfield County and Northwest Connecticut.

LPN is licensed to perform selected tasks and share responsibility under the direction of a registered nurse or an advanced practice registered nurse; and provide care within the framework of supportive and restorative care, health counseling and teaching.
LPN will provide patients with routine care as well as collaborating with the team to monitor patient conditions.
LPN will deliver care according to the appropriate scope of practice in the acute care, hospital setting.
Job Responsibilities Performs medication administration of oral agents and injections such as insulin and administer vaccines and approved IV fluids and electrolyte replacement and antibiotic administration with appropriate documentation in the electronic medical record Actively participates in quality improvement initiatives, assures all regulatory standards are met on an ongoing basis, and attends daily unit Lean Safety huddles.
Performs activities of daily living for patients Educates family and patient on treatments, procedures and medications Monitors patient and communicates changes in condition to the Registered nurse Provides point of care testing and specimen collection as directed Start and replace tube feedings at the direction of the registered nurse Administers oxygen as ordered under the direction of the registered nurse Performs EKGs as appropriate Phlebotomy trained.
Performs wound and skin care as directed by the RN Documents care provided in the electronic medical record Other duties as assigned by the registered nurse within scope of practice QualificationsEducation:
Highschool Diploma/or GED LPN Educational Training and proof of licensure in the State of CT Licensure, Certification, Registration Current Connecticut LPN License in CT BLS Knowledge, Skills and Ability RequirementsMust have demonstrated appropriate clinical knowledge to evaluate and treat patients
Experience:
